1. Preferred date for your event/party

2. Location: Are you within the City of Lethbridge, just outside the city limits, or from another community? This helps us estimate travel time.

3. Preferred inflatable (if you have one in mind)

4. Indoor or outdoor event:

   • For outdoor locations, include the surface material for the inflatable location (lawn or hard surface). Note: Gravel setups are not permitted.

5. Your email address so we can reply promptly

6. Type of event: Open to the public or private?

   • Open to the Public: Church events and similar gatherings are considered open to the public for AEDARSA certification purposes, even if not publicly advertised, because new members could join on-site.

   • Private Event: Restricted to guests on an invitation list only

     • If private, party type & size, example:

       • Child’s birthday party: Up to 2 households, with up to 25 kids as guests

       • Adult birthday, company event, family reunion, etc.: 3 or more households, with any number of guests
